Participant Agreement, Release, and Assumption of Risk
I, the undersigned, hereby voluntarily elect to participate in activities at HavFun Trampoline Park and, if applicable, allow my minor child(ren), individually and collectively referred to herein as "Child," to use the HavFun Trampoline Park facilities and equipment located at any HavFun Trampoline Park center (the "HavFun Trampoline Park Facility"). Acknowledgment of Risk at HavFun Trampoline Park: I acknowledge that participation in activities at HavFun Trampoline Park carries inherent risks, including but not limited to falls, collisions, and other potential injuries. I fully understand that these risks at HavFun Trampoline Park may result in serious injury or even death. My decision to participate at HavFun Trampoline Park is voluntary, and I accept all risks involved. Release of Liability for HavFun Trampoline Park: In exchange for being allowed to participate in HavFun Trampoline Park activities, I hereby release, waive, and discharge HavFun Trampoline Park, its owners, employees, agents, and volunteers from any and all claims, liabilities, or damages arising from my participation at HavFun Trampoline Park, even if due to negligence or other misconduct at HavFun Trampoline Park. Assumption of Risk at HavFun Trampoline Park: I acknowledge that I must adhere to all HavFun Trampoline Park rules and follow instructions provided by HavFun Trampoline Park staff. I voluntarily assume all risks associated with my participation in HavFun Trampoline Park activities, both known and unknown, and I accept full responsibility for my own safety and well-being while at HavFun Trampoline Park. Medical Treatment at HavFun Trampoline Park: I authorize HavFun Trampoline Park to obtain medical treatment for me in the event of an injury or emergency at any HavFun Trampoline Park location. I understand that I am financially responsible for any medical costs incurred due to an incident at HavFun Trampoline Park. Age Requirement for HavFun Trampoline Park: I certify that I am at least 18 years old, or that I am the parent/legal guardian of the participant who is under 18, and I am signing this agreement on their behalf to participate in HavFun Trampoline Park activities. Signature for HavFun Trampoline Park Agreement: I have carefully read and understood this HavFun Trampoline Park Participant Agreement, Release, and Assumption of Risk. I sign it voluntarily, agreeing to all terms set forth by HavFun Trampoline Park. GENERAL RELEASE for HavFun Trampoline Park From the date of signing and for all future visits of myself and my "Child" to any HavFun Trampoline Park center, I agree that this Agreement covers and provides release and legal protections to HavFun Trampoline Park, HavFun Sports and Amusement Pvt Ltd, their agents, owners, officers, managers, shareholders, affiliates, volunteers, participants, employees, and all other persons acting on behalf of HavFun Trampoline Park or HavFun Sports and Amusement Pvt Ltd.
I am at least 18 years old and I have read and agree to the terms of the above agreement.
